Our e508 submission system will allow automated processing of 508 forms required by all convicted offenders to ensure completion of DWI services before they may have their drivers’ license reinstated. The forms will be sent directly to our office, electronically, for review and approval. We will have a record of each transaction for future reporting, monitoring and evaluation.

Each agency must have a facility representative, who is given authority to access the system and maintain responsibility for agency contact information and user access. The "rep" will set up additional agency users, if needed, to complete 508 forms online.

Log In

Go to https://wirm.dhhs.state.nc.us or access directly from the web site.

  1. Type in User ID and password

  2. Click on LOGIN button

  3. Type in password given by the state

  4. Change password when prompted

    ( Remember general computer guidelines on password protection. Do not share it with others; do not leave it out on your desk, etc. The password must have 8-14 characters, lower case, and must have a number within it but not in the first position.)

  5. Click on the box labeled Certificate of Completion “Form 508”

What a Facility Representative can do

A facility rep can update information about the agency such as address changes, email, etc. This rep may also add users and determine the user’s authorization to use the system for the agency.

  1. On the left menu, click on “Update” under the heading of “DWI Facility”

  2. Choose facility from the ABC list.

  3. Click on “update”.

Make corrections to contact information as needed.

  1. Click “submit” to send changes to the state office.

To add a user

  1. Scroll down until you see “people authorized to …”

  2. click on the “add entry” button.

    Add name.

  3. Click on “look up name”.

    The system will check to see if that name is in the system already.

  4. Click on “use log on ID selected from list”.

    If email and phone are the same as the agency, leave the default check in place. If not, uncheck the default box and add an email address and phone number.

  5. Check if a facility rep.

  6. Scroll down and click on “add log on ID…”

  7. Click “submit”.

  8. Log out and Wait to hear back from state to get user ID and password approval.

  9. Then log back in.

  10. Scroll down to “people authorized to…” click on name you want.

  11. Click on “update”.

  12. Scroll down to “assessment form authorizations” and check authorizations wanted.

To Update a user

  1. Under the menu item “DWI Facility”, Click on “update.”

  2. Scroll down to “people authorized to…” click on name you want.

  3. Click on” update”.

    Add an end date

  4. If you need to cancel an employee’s access to the system.

  5. Change contact information, if needed.

  6. Scroll down to “assessment form authorizations” and Check authorizations wanted.

  7. Click on “update log on ID”.

  8. Click “submit”.

Completing a 508

  1. From the left menu click on item “508 form”.

  2. Click on section to complete (assessment, ADETS, treatment)

Assessment

  1. Enter customer number/NC license number and arrest date.

    (Be sure these are accurate-if these are not accurate a new
    form will have to be completed.)

  2. Click “Look up client”.

  3. Click “go to selected section”.

  4. Complete each item.

  5. Click “validate” to check for errors.

  6. Fix errors as needed.

  7. Then click “validate” again.

  8. Print a copy-PDF or screen text.

    (You will need the case ID number and assessment number to go to other sections of the 508).

  9. Click “Send to state.”

  10. Sign the hard copy or have it signed by appropriate person for the record.

  11. Give signed copy to the client.

For the purposes of testing, if there is a problem, you may fax signed copies of 508s to us as a back up. If the system shuts down, you may always complete a paper form as a back up, as well.

ADETS

  1. From the left menu click on item “508 form”.

  2. Click on section to complete (assessment, ADETS, treatment)

  3. Enter customer number/NC driver’s license number, arrest date.

  4. Enter DWI case ID and Assessment number (these will come from the assessment).

  5. Click “look up client”

  6. You may print a copy of the assessment here in PDF or text format.

  7. Click on “ADETS completion”

  8. Click on go to “selected section”

  9. Scroll down to the ADETS completion section.

  10. If out of state, check the out of state box.

  11. Enter date started

  12. Enter date completed.

  13. Select yes or no for fees paid in full

  14. Select yes or no for “release of information on file”.

  15. Select yes or no for appropriate signature on file approving the completion of ADETS.

  16. Click on “validate”

  17. The system will check for errors. Fix errors and click on “validate” again.

  18. Print copies either in PDF or text.

  19. Click on “send to the state office”.

  20. Sign the hard copy for the record and give a signed copy to the client.

For the purposes of testing, if there is a problem, you may fax signed copies of 508s to us as a back up. If the system shuts down, you may always complete a paper form as a back up, as well.

Treatment

  1. From the left menu click on item “508 form”.

  2. Click on section to complete (assessment, ADETS, treatment)

  8. Click on “go to selected section”

  9. Scroll down to the treatment completion section.

  10. Check the out of state box, if needed.

  11. Check level completed.

  12. Enter dates sessions started and completed.

  13. Enter fees charged.

  14. Enter fees received.

  15. Select yes or no for release of information on file.

  16. Select yes or no to indicate if you have an appropriate signature on file approving completion of treatment.

  17. Click on “validate”. The system will check for errors. Fix the errors and click on “validate” again.

  18. Make copies, click on “Print PDF” or “print text” to copy.

  19. Click on “send to the state”

  20. Sign the copies for the record and give one to the client.

User Error(s)/ Corrections

Searching for Errors:

  1. Go to the menu and click on the “assessment search” and write in the client’s name.

  2. Click on the “go to list”.

  3. Look for the status column to see where it is in the process.

    If it was rejected by DMV or approved by DMV, etc., you may be able to correct it. DMV will reject the form for the following items:
    1. customer number assigned by DMV,
    2. county of arrest,
    3. arrest date,
    4. conviction date.

  4. Click on the radio button next to the client customer number to make the correction.

  5. Click on the “update assessment” or “update the treatment” action item to make the edits.

  6. Resend it to our office and then it will go back through to DMV.

  7. If the customer number is highlighted in blue, our office has identified the DMV error for you.

If an error is made with the arrest date or customer number a new form will need to be completed. These are critical data keys and cannot be altered without creating a new form.
